Locations: Aviata: Perry, FL and Aviata North FL, Gainesville
JOB SUMMARY
We are seeking a licensed LMHC or Psychologist to provide behavioral health services across multiple post-acute care facilities. Responsibilities include conducting daily rounds, assessing and treating patients, collaborating with facility staff, and delivering accurate diagnoses, consultations, education, and follow-up care. This role involves working closely with the Psych division team within the facilities.
Applicants must hold an active Florida license or be a Registered Intern in Florida.

PRIMARY FUNCTIONS

Examine patients and document medical history
-
Adhere to established charting protocols
-
Collaborate with Physicians and other healthcare professionals to develop comprehensive treatment plans

Submit healthcare plans and patient goals for periodic physician review

Educate patients on disease prevention, hygiene, and diet
-
Refer patients to specialists as needed
-
Report deaths and contagious diseases in accordance with government regulations

Perform additional duties as assigned
EDUCATION & EXPERIENCE
-
Active and in-good-standing State of Florida license
-
Must be an LMHC/LSW in Florida

Current National Board Certification

Strong written, verbal, and electronic communication skills

Demonstrates initiative, creativity, and problem-solving ability

Ability to collaborate effectively with colleagues to ensure quality patient care
ADDITIONAL QUALIFICATIONS

Experience using electronic medical records (EMR) preferred

Bilingual (English/Spanish) REQUIRED

Reports directly to the Supervisory Psychiatric Physician / Executive Director
