Locations: Aviata: Perry, FL and Aviata North FL, Gainesville

JOB SUMMARY

We are seeking a licensed LMHC or Psychologist to provide behavioral health services across multiple post-acute care facilities. Responsibilities include conducting daily rounds, assessing and treating patients, collaborating with facility staff, and delivering accurate diagnoses, consultations, education, and follow-up care. This role involves working closely with the Psych division team within the facilities.

Applicants must hold an active Florida license or be a Registered Intern in Florida.

  • Schedule: Monday–Friday

  • Employment Type: W2

  • Hours: Flexible

PRIMARY FUNCTIONS

  • Examine patients and document medical history

  • Adhere to established charting protocols

  • Collaborate with Physicians and other healthcare professionals to develop comprehensive treatment plans

  • Submit healthcare plans and patient goals for periodic physician review

  • Educate patients on disease prevention, hygiene, and diet

  • Refer patients to specialists as needed

  • Report deaths and contagious diseases in accordance with government regulations

  • Perform additional duties as assigned

EDUCATION & EXPERIENCE

  • Active and in-good-standing State of Florida license

  • Must be an LMHC/LSW in Florida

  • Current National Board Certification

KNOWLEDGE, SKILLS & ABILITIES

  • Strong written, verbal, and electronic communication skills

  • Demonstrates initiative, creativity, and problem-solving ability

  • Ability to collaborate effectively with colleagues to ensure quality patient care

ADDITIONAL QUALIFICATIONS

  • Experience using electronic medical records (EMR) preferred

  • Bilingual (English/Spanish) REQUIRED

RELATIONSHIP REPORTING

Reports directly to the Supervisory Psychiatric Physician / Executive Director

PHYSICAL REQUIREMENTS

  • Ability to communicate effectively with patients

  • Ability to collaborate with colleagues as needed

  • Ability to operate a computer, printer, and scanner

  • Ability to sit, stand, or view a computer screen for extended periods

  • Ability to perform repetitive hand and wrist motions

  • Ability to use hands and fingers to handle or feel

  • Ability to reach with hands and arms
