POSITION SUMMARY
Freedom House Ministries is a 24/7 family shelter dedicated to walking alongside families as they work to regain stability and self-sufficiency. Through basic needs support and individualized programming, we foster an environment of hope and opportunity. Success Coaches are essential to this missionoffering trauma-informed, human-centered support that empowers families in crisis to navigate challenges and take meaningful steps toward long-term goals.
KEY RESPONSIBILITIES
Relationship Building & Intake
Coordinate screening and intake for new residents.
Conduct initial needs assessments and orient families to shelter expectations and services.
Build strong, trusting relationships grounded in respect, empathy, and accountability.
Case Management & Coaching
Develop and manage individualized, client-led case plans using Motivational Interviewing and Human-Centered Coaching strategies.
Facilitate weekly one-on-one sessions to monitor progress, reassess goals, and offer encouragement or accountability as needed.
Support families in accessing resources for employment, education, financial stability, and permanent housing.
Guide families in life skills, financial literacy, parenting, and problem-solving through group programming and direct coaching.
Data, Documentation & Compliance
Maintain thorough, up-to-date client records, including case notes, goals, and incidents.
Ensure accurate reporting in internal systems and the Homeless Management Information System (HMIS).
Collect and review documentation (e.g., pay stubs, receipts, savings records) to support financial goals and track progress.
Team Collaboration & Shelter Coverage
Work closely with internal departments, volunteers, and external agencies to coordinate services and referrals.
Participate in team meetings and contribute to program development efforts.
Provide occasional coverage throughout the shelter as needed.
Participate in team rotation of on-call duties.
Programming & Community Engagement
Assist with the planning of meaningful, inclusive programming for residents.
Model Trauma-Informed Care principles in all interactionswith clients, staff, volunteers, and community partners.
Remain engaged in professional development and ongoing training in evidence-based practices.
QUALIFICATIONS
Bachelor's degree preferred (Psychology, Social Work, Human Services, or related field). Equivalent experience may be considered.
At least 12 years of relevant case management or social services experience.
Familiarity and demonstrated application of Motivational Interviewing techniques and Human-Centered Coaching approaches.
Deep understanding of Trauma-Informed Care principles.
Excellent interpersonal and communication skills.
Organized, flexible, and able to manage multiple priorities in a fast-paced environment.
Adheres to professional Boundaries, Ethics, and the Confidentiality of those we serve.
Calm and grounded presence during crisis or conflict situations.
Strong computer literacy and documentation skills, along with Microsoft 365 experience.
