We are seeking a Part-Time/PRN Physical Therapist (PT) to join our home health team in DuPage County, IL, Specifically Elmwood Park, Roselle, Schaumburg, Medinah, Bloomingdale, Itasca, Glen Ellyn, Addison, Hanover Park, and Streamwood. This role is ideal for clinicians looking for Schedule flexibility, supplemental income, or a better work-life balance while making a real impact in patients’ homes.
You’ll evaluate, treat, and educate patients in their homes--allowing you to focus on rehabilitation without the pressure of clinic productivity standards.

What You'll Do
- Perform in-home physical therapy evaluations and treatments
- Develop and implement individualized rehabilitation plans of care
- Monitor patient progress and adjust treatment plans as needed
- Educate patients and caregivers on exercises, mobility, and safety
- Document visits accurately and efficiently in accordance with agency guidelines
- Communicate patient progress with the interdisciplinary care team
- Ensure compliance with clinical, documentation, and regulatory standards
What We're Looking For
- Licensed Physical Therapist (PT)
- Doctorate in Physical Therapy (DPT)
- Home Health experience preferred, but not required
- Strong communication and documentation skills
- ability to work independently in a home-based setting
- professional, dependable, and patient-focused approach
- Physical ability to perform in-home visits
