Job Description:\n\nSalary: $15.00 - $18.00 per hour DISCLOSURE - MULTI-EMPLOYER ROLE: This W-2 hourly Direct Support Professional position includes responsibilities that span across the following two affiliated organizations, all under shared leadership: FOCUS #1Community Wellness Psychiatric Rehabilitation Program FOCUS #2HolBrock Estates Supportive Housing Programs You will maintain separate W-2 employment relationships with each organization and will receive individual payroll compensation from each entity based on the time worked and services provided for their respective programs. This means you will be an official employee of all three companies, with hours, responsibilities, and compliance obligations tracked separately for each. This role is structured as an up to 24-hour per week position, with approximately 12 of the total weekly hours allocated to Community Wellness Psychiatric Rehabilitation Program and 12 of the total weekly hours allocated to HolBrock Estates Supportive Housing Programs. Client Service Location Expectations: This position is based on-site at the supportive housing program located at 3310 Eastern Avenue, 21224, MD 21205. All services are to be delivered in person at this location. The staff member is expected to be present on-site during scheduled shifts, provide support within the residence, and ensure documentation and responsibilities are completed in alignment with program protocols. Remote work or off-site service delivery is not applicable for this role. Caseload Makeup: The assigned caseload for this position consists of four (4) Category A clients who reside in the community and may be serviced at 3310 Eastern Avenue, Baltimore, MD, throughout the community, or in the community meeting room of 809 N Rose Steet location. DIVISION: Operations CLASSIFICATION: This is a part-time, W-2 employee position totaling 24 hours per week, with hours allocated across three affiliated organizations as follows:HolBrock Estates Supportive Housing Programs (HESHP):Part-time, W-2 employee approximately 12 hours per week (50% of total weekly hours)Community Wellness Psychiatric Rehabilitation Program (CWPRP):Part-time, W-2 employee approximately 12 hours per week (50% of total weekly hours) While each assignment is classified as part-time under its respective entity, the combined total constitutes a part-time, 24-hour per week role across the organization network. Through individualized service planning, case management, health home coordination, and peer support, the PRP helps clients navigate housing, employment, education, and health care access. The DSP helps clients build daily living skills, access resources, follow through on treatment goals, and develop coping strategies that support their wellness and recovery.
