Shift: Evening – 8-Hour (3:00 PM – 11:30 PM) or 12-Hour (11:00 AM – 11:00 PM)
Pay Rate: $45 per hour

Job Description:

We are seeking a dedicated and experienced Licensed Practical Nurse (LPN) to join a well-established Long Term Care facility in Watertown, NY on a 13-week contract. This role offers flexible evening shifts (either 8 or 12 hours) and the opportunity to provide compassionate care to residents in a supportive and collaborative environment.

Key Responsibilities:

• Provide high-quality nursing care to residents with chronic and complex conditions, including dementia, diabetes, and other long-term illnesses
• Administer medications and treatments accurately, using PointClickCare (eMAR)
• Collaborate with the interdisciplinary healthcare team to ensure individualized resident care plans are followed
• Monitor patient health, document observations, and respond to changes in condition
• Assist with ADLs (activities of daily living) and support residents' overall well-being
• Comply with all regulatory standards, safety guidelines, and facility protocols

Schedule & Work Conditions:

• Shift Options:
  • 8-Hour Evenings: 3:00 PM – 11:30 PM
  • 12-Hour Evenings: 11:00 AM – 11:00 PM
• Contract Length: 13 Weeks
• Must be willing to work occasional weekends and holidays as needed

Requirements:

• Active LPN license in New York State
• 1–3 years of recent experience in LTC or Skilled Nursing Facilities (SNF)
• Current BLS certification (AHA)
• Experience with PointClickCare eMAR preferred
• Strong communication and clinical assessment skills

Experience with PointClickCare eMAR preferred\n\nTwo shift options available 5x8's and 3x12's (rates affected respectively).\n\n1–3 years of recent experience in LTC or Skilled Nursing Facilities (SNF)
