Responsibilities
About the Role
During visits that may last up to one hour, our providers assess and observe a member’s health status, medical history, medication adherence, social circumstances, and other potential risks. This approach provides invaluable insights into a member’s overall health that are often missed during standard office visits. The Matrix Comprehensive Health Assessment (CHA) enhances care quality and allows us to address multiple care gaps in a single appointment. Our providers are responsible for:
- Conducting assessments for adults and geriatrics, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and thorough documentation in line with our protocols.
- Administering and ordering necessary point-of-care and lab-based tests.
- Collaborating with physicians, case managers, social workers, family members, key caregivers, and other medical personnel as needed.
- Working alongside Primary Care Physicians (PCPs) to provide patient education and follow-up as required.
- Delivering services at the member’s residence, which may include their homes or Skilled Nursing Facilities.

- Flexibility – Schedule visits according to YOUR availability. Assessments are conducted from Sunday to Saturday, between 8 am and 7 pm. You can choose to work during the day, evening, or weekends based on your preferences.
- Competitive Pay – Your earnings are based on the number of assessments completed. Increase your income potential by being available for more visits and traveling to see members outside your home area. Those averaging 20 to 100 visits monthly can earn between $1,800 and $11,500 per month, depending on the assessment completion rate of $90 to $115, based on your state of residence.

- Education and Support – We prioritize your development! Along with comprehensive onboarding training, you will be assigned a preceptor, receive personalized feedback, and gain free access to continuing education opportunities.
- Additional Licensure – If business needs arise, we will assist you in obtaining additional state licensure and credentialing in neighboring states or others you wish to visit while conducting health assessments.
- Full-time Opportunities – Our PRN providers have the option to transition to full-time roles supporting their home location or a specific state or region, along with a full-time sign-on bonus.
Qualifications
Skills & Experience That Shine
- A Master’s Degree or equivalent experience with successful completion of NP or PA licensure.
- Current NP or PA licensure in the state of practice, including prescription authority or the ability to obtain it.
- Board certification from NCCPA, AANP, ANCC, or AACCN in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
- Current BLS, ACLS, or CPR certification.
- Preferred one year of experience as a Nurse Practitioner or Physician Assistant; new graduates are encouraged to apply!
- A valid state driver’s license and proof of adequate automobile insurance for the state of residence.
- Strong computer skills and familiarity with health/medical record software.
- Excellent verbal and written communication abilities.
