Position Details:

Monday - Friday, 8:30am-2:30pm, 30 hours per week/Full-Time

$19.38 per hour

Primary Purpose:

To provide direct care in a day program setting to participants with special needs to enable their highest quality of life in terms of their physical, emotional and mental health while adhering to CCARC policies and procedures. This role involves working closely with the arts faculty to provide support to participants in the art internship program. The Art Internship DSP will facilitate the use of specialized equipment and technology, help complete basic classroom tasks, teach self-care skills, and assist art curriculum instructors with achieving internship learning outcomes in painting, drawing, and other art forms.

Essential Functions:

  • Provides and receives effective, respectful, appropriate, and courteous verbal and written communications to and from specific teams related to the care of participants, including daily observations, reports of any physical or behavioral changed while adhering to all documentation and record keeping requirements.
  • Develop, implement and maintain neat, accurate and comprehensive written assessment records, annual reviews and biannual or quarterly reviews as needed, Community or Vocational Service plans, approved Behavior Programs and Procedures or any other records, ensuring confidentiality of all information
    • Provides essential physical and emotional care to participants including;
  • Assisting with personal care, hygiene, and treatments including administration of medications, recording vital signs as needed
  • Providing education to participants in personal hygiene, grooming, and behavioral conduct while acting as a role model to enhance participants’ self esteem
  • Coordinating and leading social and recreational activities both within the community as well as at the main facility. This includes the safe transportation of participants
  • Maintain cleanliness of program spaces
  • Nutritional support: Monitoring individuals at mealtime according to food consistency guidelines for each individual
  • Attentive to the safety of all individuals, self and others
  • Ensure the proper use of equipment, facilities and vehicles. Report all incidents and submit appropriate incident reports
  • Demonstrates knowledge of emergency procedures and responds appropriately to emergency situations
  • If supporting individuals in a work program;
    • Supervise and guide participants through curriculum activities, supporting their acquisition of knowledge and skills aligned with the internship learning outcomes.
    • Train, supervise and support individuals in order for individuals to sustain employment, including adapting equipment/activities in order to maximize the potential of the individuals working
    • Evaluate the individual’s appropriateness, desires, strengths and abilities for employment
    • Act as a primary liaison person between any employment or volunteer site and CCARC promoting the individual’s inclusion at the site(s) and developing and maintaining all pertinent records related to the site(s)
  • Demonstrates adherence to universal precautions, OSHA standards and infection control policies and procedures at all times. Also, collaborating to resolve conflicts, obstacles, and challenges, seeking guidance from manager and team members, as needed
  • Assist in training and support of all substitute staff
  • Review and be familiar with information in master files and correctly file all appropriate documents in the master files
  • Maintain good daily attendance (days of absence, illness, tardiness)
  • Attends and participates in mandatory Training, in-services, staff meetings and conferences as required

Education and/or Work Experience Requirements:

  • High School Diploma / GED
  • Must have valid Driver's License, be insurable with Agency carrier. Individual Home Support employees are required to have a vehicle for use and maintain adequate insurance
  • Must complete the Agency’s Employee Training
  • Certification and training, including but not limited to: CPR, First Aid, PMT (Psychological/Physical Management Training) and DDS Medication Administration
  • Ability to follow oral and written instructions
  • Experience with Microsoft products, use of email and Therap preferred

  • Persons in this job must possess the physical and emotional health needed for effective and efficient completion of duties
  • Individuals in this job are likely to be required to perform lifting and transferring activities and may be required to restrain residents/program participants; may be exposed to risk of injury from abusive/assaultive residents
