JOB TITLE: Team Lead

Bargaining Unit Status: Non-Bargaining Unit

FULL-TIME ☒ (hours per week: 40) WORK SCHEDULE: 7:00 AM-4:00 PM M-F

EDUCATIONAL QUALIFICATIONS:
Bachelor’s degree from an accredited college in rehabilitation, counseling, psychology, sociology, social work, or a related discipline (a combination of equivalent education and work experience may be considered).

EXPERIENCE QUALIFICATIONS:

  • Three or more years’ experience in a rehabilitation, non-profit facility, or comparable setting in the area(s) of vocational rehabilitation, substance abuse, mental health counseling, service delivery, or case management
  • Proficiency with the Microsoft Office suite and general computer operation is required
  • Working knowledge of basic case management principles.
  • Strong working knowledge of disability management and awareness.
  • Additional Preferred Qualifications: Broad knowledge base of disabilities and issues influencing those with various conditions.

OTHER QUALIFICATIONS:

  • Must be authorized for coverage under agency insurance if operating an agency vehicle, have a good driving record, a valid driver’s license, and/or the required level of personal auto insurance.
  • Must be able to satisfactorily complete a background check, per company requirements.
  • Ability to communicate with tact and maintain confidentiality.
  • Adequate ability in communicating in spoken conversation – adequate volume of speech.
  • Adequate ability in writing/reading/mathematics.
  • Ability to effectively build and maintain positive working relationships with management and peers.
  • Strong organizational, project planning, and management experience with proven analytical and multi-tasking skills.
  • Superior listening, verbal, and written communication skills.
  • Proficiency in general reporting and basic case file management.

IMMEDIATE SUPERVISOR:
Program Coordinator and/or Director of Vocational Services

POSITIONS SUPERVISED:
Provides functional support to team members but does not serve in a direct supervisory role

JOB RESPONSIBILITIES SUMMARY:
Provides day-to-day operational support for the team, helping to ensure service continuity, timely documentation, and internal communication. Acts as a liaison between staff and leadership while maintaining their caseload and supporting a positive team environment. Participates in leadership development through collaborative communication, mentoring, and coordination activities. Continues to provide vocational services to individuals with disabilities, ensuring they remain gainfully employed. Maintain communication with referral agencies, family members, workplace supervisors, and other involved parties. Remain compliant with AbilityOne and CARF regulations and adhere to company policies related to service provision.

ADDITIONAL RESPONSIBILITIES
Serve as a Human Resources Coordinator as needed. This would include, but is not limited to, assisting with onboarding documents, handling basic employment-related inquiries from employees, and referring employees with more complex or sensitive matters to the appropriate HR contact at headquarters.

ESSENTIAL FUNCTIONS:

  • Have working knowledge of current contract and referral source requirements as well as CARF standards, DOL, social security guidelines, and Phoenix mission and internal policies.
  • As necessary, develop, write, and present progress reports and discharge summaries documenting performed activities.
  • Complete training as defined by the Phoenix Professional Development Plan.
  • Represent Phoenix in the business community by involvement in civic and professional organizations.
  • Provide information as requested for various statistical requirements.


SPECIFIC RESPONSIBILITIES RELATED TO THIS POSITION MAY INCLUDE ANY OF THE FOLLOWING:

Team Lead: Educational Services, Partners with Industries, Milestones

  • Oversee and support the daily operational needs of the team, including coverage coordination, staff guidance, and urgent response assistance.
  • Maintain a caseload and complete all related documentation in compliance with standards and timelines.
  • Serve as the first point of contact for staff on urgent matters and communicate with Program Coordinator/and or Director of Vocational Services immediately as needed.
  • Conduct check-ins with the Program Coordinator and/or Director of Vocational Services multiple times per week for updates, feedback, and planning.
  • Facilitate internal communication and help ensure information flows clearly between staff and leadership.
  • Coordinate and implement morale-building efforts such as shout-outs and team-based activities
  • Lead or shadow internal meetings to support leadership development and understanding of organizational systems
  • Mentor new staff members and assist with onboarding and training in collaboration with the Program Coordinator and/or Director of Vocational Services.
  • Identify and communicate opportunities for process improvements or service enhancements to the Program Coordinator/Director of Vocational Services.
  • Create and maintain shared team tools and resources to improve team efficiency.
  • Strengthen cross-team communication and collaboration by promoting effective information sharing and consistency in service delivery.
  • Manage or monitor team metrics, including billing accuracy, documentation timeliness, calendar coordination, and progress report submission.

  • Adequate ability for conversation, telephone use, and emergency information systems.
  • Ability to be independently mobile on all surfaces and situations.
  • Ability to move safely about the work area.
  • Ability to retrieve items from high and low areas.
  • Ability to perceive size, discrimination, temperature, shapes, and/or textures.
  • Physical stamina to work at least 8 hours per day.
