About Us

Our organization has been a pioneer in establishing the first national in-home clinical network. Today, we operate as an independent provider of thorough health assessments, catering to individuals nationwide. We are dedicated to delivering exceptional, personalized care that enhances health, reduces costs, and empowers individuals to lead fulfilling lives. With a strong foundation in clinical assessment and care management, our extensive network of clinicians transcends traditional barriers by meeting members in their own environments. We assist older adults and other at-risk populations in achieving better care quality, improving health outcomes, and identifying chronic conditions that might otherwise remain undetected.

Joining our team allows you to utilize your skills and compassion to make a meaningful impact on the health and well-being of others. We offer competitive base salaries along with incentive compensation and benefits while you contribute positively to the community!

Responsibilities

About the Role

During visits that may last up to one hour, our providers assess and observe a member’s current health status, medical history, medication adherence, social circumstances, and other potential risks. This approach provides invaluable insights into a member’s overall health that are often missed during standard office visits. The Matrix Comprehensive Health Assessment (CHA) enhances care quality and allows us to address multiple care gaps in a single appointment. Our providers are responsible for:

  • Conducting assessments for adults and geriatrics, including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and thorough documentation in line with our guidelines and protocols.
  • Administering and ordering necessary point-of-care and lab-based tests.
  • Collaborating with physicians, case managers, social workers, family members, key caregivers, and other medical personnel as needed.
  • Working alongside Primary Care Physicians (PCPs) to provide patient education and follow-up as required.
  • Delivering services at the member’s residence, which may include their homes or Skilled Nursing Facilities.

Benefits of Working with Us

  • Consistency – While assessments are conducted from Sunday to Saturday between 8 am and 7 pm, our full-time providers generally work Monday to Friday from 8:00 am to 5:30 pm.
  • Competitive Pay – Enjoy an annual base salary ranging from $95,500 - $158,400, determined by market analysis and experience.
  • Incentive Program – Includes uncapped monthly incentives and year-end bonuses.
  • Comprehensive Benefits – Mileage reimbursement, medical, dental, vision, life insurance, PTO accrual, 401(k), paid holidays, continuing education time/reimbursement, and employee referral bonuses.
  • Education and Support – We prioritize your development! Along with extensive onboarding training, you will be paired with a preceptor, receive personalized feedback, and gain free access to online continuing education opportunities.
  • Additional Licensure – If business needs arise, we will assist you in obtaining additional state licensure and credentialing in neighboring states or others you may wish to visit while conducting health assessments.
