SUMMARY: The Adult Day Support Professional position is a non-exempt, direct care position responsible for self-help skills, social skills, employment skills training, personal care, and behavior support to adults with developmental disabilities. This is a direct service position, which requires hands on work with individuals with severe disabilities who may be developmentally and behaviorally challenged.
ESSENTIAL FUNCTIONS:
❖ Provides needed support to everyone:
o Responsive to all needs of individuals (communication, personal care, social, behavioral supports, etc.)
o Actively promotes/ensures individuals’ rights o Actions reflect Tri-More’s vision, mission and philosophy
o Acts as a role model and provides positive reinforcement
❖ Implements and documents all program components while adhering to agency policies/procedures
o Provides Active Treatment services to individuals where required
o Implements/documents services and supports, behavior plans, and skill development goals as identified in each person’s Individual Plan
o Documents Billing Forms, Unusual Incidents, Vehicle Travel Sheets
o Completes formal and informal assessment of the needs of individuals to ensure all needs are included in the Individual Service Plan
o Assists individuals with making purchases and maintains receipts in accordance with established procedures
o Review and update each client’s individual plan, on a semi-annual basis, or if there is a significant change in the individual’s status
o Assist in the development of goals and objectives for each client
❖ Plan and Implements day program activities
o Assist with planning activity programs that meet the individual needs of each client
o Help develop a monthly calendar of activities and events
❖ Ensures the health and welfare of consumers:
o Attends to medical/health needs:
o Observes for signs and symptoms for illness or injury, ensuring individual receives needed evaluation or treatment
o Ensures adherence to special diet needs (allergies, diets, modified textures, etc)
o Performs Delegated Medication Administration accurately
o Provides basic first aid and/or CPR in accordance with training
o Assists individuals in maintaining good grooming and a presentable appearance
o Assists individuals with personal care as needed per their individual service plan
o Assist individuals with maintenance therapy to maintain optimal functioning and to prevent regression under the direction of a therapist, physician, or registered nurse
❖ Ensures safety and maintains a sanitary environment:
o Ensures the day support environment is clean, organized, and safe
o Food is handled and prepared in accordance with safety guidelines
o Ensures individuals’ safety regarding fire and severe weather; conducts training and drills routinely
o Ensures safety in the community (pedestrian, vehicle, around pools/lakes/streams, etc.)
o Ensures proper and safe use of different types of equipment used in various programs
❖ Operates Non-Medical Transportation vehicles (12 passenger vans) in accordance with ODMV rules and drives defensively
o Completes inspection of vehicle in accordance with policies and procedures ensuring vehicle is safe to operate
o Ensures all passengers are safely secured in appropriate passenger/wheelchair restrain devices
o Reports unsafe conditions to supervisor in a timely manner
❖ Interacts well with others
o Works effectively and cooperatively with all team members treating them with courtesy and respect
o Ensures all information regarding the individual's care and services is communicated professionally to individuals, their families/advocates, and other service providers responsible for the care of the individual
o Attends and actively participates in meetings/trainings as directed and in compliance with policies/procedures o Oral and written communication is professional, courteous, clear and concise
o Fosters confidentiality in all communications
