Responsibilities:
- Provide clinically competent care for home dialysis patients, including assessment, planning, intervention, and evaluation.
- Perform systematic collection and analysis of dialysis data and document findings in patient medical records.
- Collaborate with patients and families to assess learning needs and provide education.
- Direct and provide all aspects of safe and effective dialysis therapy for assigned home patients.
- Administer prescribed medications and document medical justification and effectiveness.
- Assist with emergency response measures as necessary.
- Serve as a resource for healthcare team members, participate in staff training, and orient new staff.
- Ensure proper laboratory collection, processing, shipping, and rescheduling of lab collections as needed.
- Maintain accurate nursing assessments and care plans in collaboration with the Interdisciplinary Team.
- Ensure patient awareness regarding transplant and treatment modality options.
- Identify, evaluate, and educate Home Dialysis candidates and their home care partners.
- Perform home environment assessments to ensure readiness for home dialysis treatments.
- Train patients and home partners on equipment operation, treatment protocols, and safety procedures.
- Participate in on-call rotations, weekends, holidays, and off-hour coverage as needed.
- Travel to training sites, patient homes, and other facilities as required.
Physical Demands and Working Conditions:
- Regular heavy lifting and moving of patients and equipment (up to 200 lbs. with assistance).
- Frequent standing, bending, and lifting up to 30 lbs.
- Exposure to infectious materials and healthcare facility environments.
- Desk work, computer work, and interaction with patients, families, and healthcare staff.
- Must complete Ishihara’s Color Blindness Test (accommodations available if necessary).
- May be required to travel for patient care and training responsibilities.
Qualifications:
- Graduate of an accredited School of Nursing.
- Current state RN licensure.
- Current CPR BLS Certification.
- At least 1 year of nephrology nursing experience within the last 2 years.
- Minimum of 1 year experience in home dialysis therapies (peritoneal and/or hemodialysis).
- Minimum of 12 months experience as a Registered Nurse (RN).
- Successful completion of all required education and training for home dialysis care.
- Ability to provide supervision to other clinical staff as assigned.
