Part-Time / As-Needed (Hourly + Monthly Stipend)

Creative Home Health, a division of Florry Creative Care Corp., provides compassionate, patient-centered home health services throughout Central Florida. We areseekinga skilled and motivatedOccupational Therapist (OT)to provide in-home therapy services to our clients on an as-needed basis.
This position offers the flexibility of per-visit scheduling whilemaintainingemployee status (W-2), including applicable payroll tax contributions and agency support.
Position Summary:
The Occupational Therapist evaluates and treats patients in their homes, focusing on improving their ability to perform activities of daily living and promoting independence, safety, and overall quality of life. The OT also provides education to patients and caregivers to support long-term functional outcomes.This role may include a
monthly stipendto ensure ongoing readiness, documentation compliance, and availability.
Key Responsibilities:
- Perform occupational therapy evaluations in the home setting
- Develop and implement individualized treatment plansin accordance withphysician orders
- Address deficits in activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
- Train patients in adaptive techniques, compensatory strategies, and use of assistive devices
- Recommend home modifications to improve safety and functional independence
- Educate patients, families, and caregivers on therapeutic activities and safety strategies
- Monitor patient progress and adjust treatment plans as needed
- Complete all required documentation accurately andtimelyper agency and regulatory standards
- Communicate effectively with physicians, case managers, and interdisciplinary team members
- Maintain compliance with all federal, state, and agency policies and procedures
Qualifications:
- Current Florida Occupational Therapist license in good standing
- Graduation from an accredited Occupational Therapy program
- Home health experience preferred but notrequired
- Strong organizational, communication, and clinical decision-making skills
- Ability to work independently in the field
- Valid drivers license and reliable transportation
- Current CPR certification
- Ability to pass background screening and drug testing
