Job Overview
We are seeking a dedicated and compassionate Physical Therapist to join our team in Jackson, CA. The ideal candidate will have a strong background in patient care, a passion for helping individuals regain their mobility, and a desire to improve their overall quality of life. In this role, you will work directly with patients in their home environments, providing personalized, one-on-one therapy to enhance recovery, promote independence, and improve physical function. You will play a key role in helping patients achieve their rehabilitation goals, ensuring they receive the highest level of care while fostering a supportive environment for healing.
Responsibilities
- Conduct Comprehensive Patient Assessments: Perform initial and ongoing evaluations of patients to assess physical capabilities, limitations, and functional goals, taking into consideration medical history, diagnoses, and current condition.
- Develop and Implement Treatment Plans: Create personalized, evidence-based treatment plans tailored to each patients needs, focusing on improving mobility, strength, and balance.
- Therapeutic Interventions: Apply a variety of therapeutic modalities and exercises to address patient conditions, such as strength training, balance exercises, mobility improvement, and pain management techniques.
- Patient Education and Family Support: Provide education to patients and their families about the treatment process, home exercise programs, and safety protocols. Offer strategies to manage pain, prevent falls, and promote self-care outside of scheduled therapy visits.
- Monitor and Adjust Treatment Plans: Continuously evaluate patient progress, modifying treatment plans as needed to ensure the best possible outcomes. Collaborate with patients, families, and the healthcare team to make adjustments to care as necessary.
- Documentation and Compliance: Maintain detailed and accurate records of all patient care activities, treatment interventions, and progress notes. Ensure all documentation complies with HIPAA and state regulations.
- Team Collaboration: Work closely with other healthcare professionals, including physicians, nurses, occupational therapists, and social workers, to provide holistic care and ensure patients needs are met.
- Continuing Education: Stay up to date with the latest physical therapy techniques, industry standards, and best practices by participating in ongoing professional development opportunities.
Experience and Qualifications
- Educational Requirements: A degree in Physical Therapy from an accredited institution.
- Licensure: Must hold a valid state licensure as a Physical Therapist in California, or be eligible for licensure.
- CPR Certification: Current CPR and BLS certification required.
- Experience in Home Health: Previous experience in home health is preferred, but not mandatory. Candidates with strong clinical backgrounds in other settings are encouraged to apply.
- Communication Skills: Exceptional communication skills to work effectively with patients, their families, and interdisciplinary team members. The ability to educate and inspire trust is essential for patient success.
- Knowledge and Skills: Comprehensive knowledge of physical therapy practices, medical terminology, and evidence-based interventions. Ability to develop and adapt treatment plans based on a variety of patient needs and conditions.
- Physical Requirements: Ability to lift, carry, and support patients as necessary. Comfortable working in a variety of home settings, including navigating stairs and handling uneven terrain.
