Job Summary:
This is a grant funded position that reports to the Clinical Support Manager. The Patient Navigator is responsible for promoting and managing cancer screenings and enrollment assistance to improve access and adherence providing service linkage to address other psychosocial needs as appropriate. ?Provides customer service, support, education, outreach, and case management to patients consistent with the grant requirements. Working interactively with internal teams, patients, healthcare providers, pharmacies, and payers.
Requirements:
Responsibilities: *Include but are not limited to the following
- Serve as critical resource for patients across the patient care team to facilitate ease of and satisfaction with the patient experience and treatment cycle.
- Identify the proper candidates for cancer screenings and assist in guidance and coordination of screenings.
- Act as a liaison between the patient and family with the multidisciplinary healthcare providers to ensure effective communication and collaboration.
- Support patient enrollment in the Breast and Cervical Cancer Project (BCCP) program by identifying eligible individuals, explaining program benefits, and assisting with the application process.
- Provide targeted health education to patients and community members regarding cancer prevention, screening guidelines, treatment options, and available support services, including BCCP resources.
- Collaborate with grant partners, internal teams, and leadership to conduct outreach and marketing activities that promote oncology services and BCCP participation. Conduct outreach efforts within designated communities to help increase the number of cancer screenings among Medicaid and high-risk patients and ensure patients follow up with their care.
- Develop relationships with referring health care providers and ensure the complete and efficient management of all referrals.
- Track all patient contacts, follow-ups, barriers to care, and outcomes, with special emphasis on oncology patients and BCCP program metrics. Review medical records to determine if follow up care or imaging was completed, and facilitate next steps as needed
- Utilize EPIC EMR system to ensure patient records are up-to-date and secure.
- Compiles data, tracks outcomes, and complete required program reporting
- Makes recommendations for process improvement, including but not limited to oncology navigation, cancer screening coordination, and BCCP program efficiency.
- Manage the required set of grant documents and reporting information, in conjunction with leadership.
- Collaborates with managers, and agencies regarding outlined plan of care; assists with referrals, problem solving or overcoming obstacles to enable patients to achieve optimal level of functioning.
