<li>Represents MOG Home Health Care in a professional manner to families, service coordinators, State officials, and community partners when required;</li>

TEAMWORK
<ul>
<li>Assists the Direct Support Supervisor in the preparation for the annual Individual Planning Meeting;</li>
<li>Treats all individuals and coworkers and with dignity, respect, and fairness;</li>
<li>Provides support to other team members when individuals are having behavioral issues;</li>
</ul>
DOCUMENTATION
<ul>
<li>Documents all activities in individual service notes and update the activity charts when required; </li>
<li>Obtain individuals health documents and provide copies to MOG (i.e. physical, discharge documents, doctors notes, appointments, specialist recommendations,);</li>
<li>Obtain health related education documents from Direct Support Supervisor to educate individuals and families</li>
<li>Attends health fairs and other trainings, meetings that are scheduled for the benefit of individuals</li>
<li>Ensures that all documentation is completed accurately and in a timely manner;</li>
<li>Clocks in and out before and after every shift and submits Leave Request forms when applicable</li>
</ul>
PHYSICAL REQUIREMENTS:
<ul>
<li>Visual/hearing ability sufficient to comprehend written/verbal communication.</li>
<li>Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing.</li>
<li>Ability to lift/push/pull 50lbs. </li>
<li>Ability to work on holidays and during inclement weather </li>
<li>Is neat in appearance and practice, with good personal hygiene</li>
