Responsibilities
<ul>

<li>Ensure individuals' safety and maintain a safe living environment.</li>
<li>Promote communication, choice, independence, and socialization.</li>
<li>Light housekeeping tasks/ running errands/ meal/snack preparation when it is needed.</li>
</ul>
Experience
<ul>
<li>Must be able to pass Local and Level 2 Background in AHCA. This is an employee position with APD CDC+ program.</li>
<li>Spanish language is required.</li>
<li>Knowledge of Positive Behavior Support.</li>
<li>Previous experience working with individuals with developmental disabilities is desirable.</li>
</ul>

Job DescriptionFamily Transitions, Inc. is looking for a qualified, and compassionate DSP (Direct Support Professional) to fill positions specifically for the Petersburg area. Day Time Hours: 8:30AM until 3:30PM Monday through Friday. DUTIES AND RESPONBILITIES: 
<ul>
<li>Assists or inspects individuals on ADL's (Activities of Daily Living).</li>
<li>Provides care to individuals as defined in their ISP (Individual Service Plan).</li>
<li>Administers medication as necessary.</li>
<li>Monitor special conditions, i.e. seizures, diabetes, special diets, physical disabilities, and dual diagnosis.</li>
<li>Transports individuals to all activities as specified by supervisor.</li>
<li>Completes all required individual related clinical documentation including progress notes, incident reports, and data tracking according to Family Transitions, Inc. Health, Safety and Emergency guidelines.</li>
<li>Recommends revisions be made on treatment goals to better serve the individual's specific needs.</li>
<li>Intervenes and responds to a crisis following Family Transitions, Inc.'s policies and procedures.</li>
<li>Performs/assists in household maintenance to sustain a safe and positive environment.</li>
</ul>
KNOWLEDGE, SKILLS, AND ABILITIES: 
<ul>
<li>Ability to communicate effectively, both orally and in writing.</li>
<li>Knowledge of the special needs of individuals with developmental disabilities, intellectual disorders, and mental illnesses.</li>
<li>Ability to analyze needs of verbal and non-verbal individuals.</li>
<li>Working knowledge of human service system, roles of human services personnel, and relevant community resources.</li>
<li>Skilled in detailed, precise, and grammatically correct documentation.</li>
</ul>
QUALIFICATIONS: 
<ul>
<li>High School diploma or equivalent.</li>
<li>1 year experience working with adults who have developmental/ intellectual disabilities.</li>
<li>Valid Driver's License is required.</li>
<li>Must have a positive driving record and proof of automobile insurance.</li>
<li>Must pass a Criminal Background Investigation.</li>
</ul>
ADDITIONAL INFORMATION: 
<ul>
<li>Applicants will be subject to a check of the Department of Social Services (DSS) Central Registry of child abuse and neglect complaints.</li>

 GENERAL SUMMARY: (General Purpose of the Job) Performs duties as delegated by Program Manager in licensed community residential facility for persons with intellectual disabilities or behavioral health.
 
ESSENTIAL FUNCTIONS: (Primary Responsibilities) Some duties and responsibilities vary in intensity and frequency depending on the program.
Provide a safe, clean, comfortable and therapeutic living environment for persons in a community residential setting. Encourage, guide and train individuals in developing daily living skills in a community residential setting. Assist individuals in maintaining their physical health and hygiene. Medication Administration Assist in organizing and participating in leisure/recreation activities. Demonstrate and coach the individuals using approved techniques and strategies. Participate in the maintenance of overall operation of a community residential setting. Follow Agency policy and procedures. To participate in the ongoing communication regarding program planning and implementation for individuals. 
 
QUALIFICATION STANDARDS: High School Diploma or equivalent (as per program requirements) 18 years or older - 21 years or older Personal Care Homes Pass and maintain Medication Certification standard Valid U.S. Drivers License and able to be insured on company policy
