OVERVIEW The Resident Relocation Coordinator is a key member of the Relocation team. The Resident Relocation Coordinator is responsible for overseeing the daily relocation tasks required at multiple sites while working closely with residents to prepare and assist them with all required relocation activities. The position involves regular communication with clients, managing third-party vendors, including cleaners, moving and pest control companies, and data collection and tracking. The Resident Relocation Coordinator represents HOU at company events and helps move our mission and reputation forward.
RESPONSIBILITIES ●Conduct site setup activities, including the arrangement of office, relocation files organization, identification of vendors, and off-site relocation units. ●Participate in surveying residents to understand their relocation needs and relocation preferences. ●As part of the assessment process, take an inventory of tenant belongings and furniture, taking note of large furniture, infestation, clutter, and hoarding issues. ●Participate in and facilitate resident and community meetings and drop-in sessions to discuss the relocation process and resident relocation rights and benefits. ●Prepare and distribute appropriate notices to affected households regarding the relocation program and ensure proof of resident receipt for notices are received and on file as required. ●Provide relocation counseling and assistance in compliance with applicable Federal, State, or local regulations. ●Implement relocation policies as outlined in the approved relocation plan. ●Serve as a liaison between HOU, property management, clients, and residents. ●Provide weekly and monthly reports to clients, the project manager, and the development team. ●Regularly update and maintain relocation files, database, lottery list, and relocation tracking reports. ●Create an agenda for weekly relocation meetings, take minutes of each meeting, and distribute them to the team within 48 hours of the meeting.
●Conduct pre-inspection of units to ensure they meet decent, safe, and sanitary requirements before offering them as a comparable relocation unit. ●Notify residents of the move-in date and provide ongoing contact and support to ensure they are prepared to move. ●Check-in with residents at least 30 days before moving and follow up weekly three weeks before the moving date. Conduct final check-in 72, 48, and 24 hours before the moving date. ●If needed, provide packing supplies, tips, demonstrations, and assistance. ●Assist with utility disconnection and reconnection and completion of the change of address forms (if applicable) for households needing assistance. ●Schedule, coordinate and supervise moves and other move-related tasks, including pest inspection and unit turnover as needed. ●Work closely with construction, property management, development, and moving contractors to ensure residents vacate their units per the relocation schedule, surrender keys to the property management, and units are properly secured. ●Implement procedures for minimizing resident property damage, including inventorying furniture and taking photos or video, and serve as the initial point of contact regarding resident claims of damage or loss. In the event of property damage or missing belongings, serve as a liaison between the resident and moving company to facilitate timely resolution of the claim. ●Interface with residents' families, friends, or social services providers to ensure seamless delivery of services and address relocation challenges. ●Serve as an HOU representative to local, state, federal, housing agencies, and community groups. ●Participate in weekly supervision and scheduled site meetings with the Project Manager/Assistant Project Manager. ●Ensure relocation files contain required documentation and the data-tracking system is updated regularly. ●Complete all tasks identified in the site work plan and perform other duties as assigned.

These communities are operated by TELACU Residential Management, Inc. (TRM), which provides the highest levels of in-house oversight, security, maintenance and social service care that our residents deserve. Each community is managed with the philosophy that we are servants in their home, rather than them being guests in ours.
The Resident Services Coordinator (“RSC I”) is responsible for providing supportive service to elderly and physically challenged residents living at TELACU Residential Management (“TRM”) housing communities. RSC Level I to Level II requires evaluation of employee skill and performance levels by the Director of Resident Services Programs (DRSP) and Resident Services Supervisor (RSS). Due to the nature of the work assigned, the timeframe and transition will vary, and in some instances, an RSC Level I may be permanently allocated an RSC Level I.
The RSC I, works with residents in a comprehensive and proactive manner to assess and identify resident needs and implements individual health-driven aging plans and educates residents on available resources. The position is an advocate for residents, manages the 504 reasonable accommodation process, and works on case management to prevent premature institutionalization to supports residents to safely age in place. 
Responsible for creating partnerships with social services partners and health care providers facilitating care management meetings to ensure situations are coordinated across partner organizations while the aggregate need of the resident is supported through appropriate services and helps with transition plans in collaboration with hospitals, assisted living, rehabilitation facilities, nursing homes and impacted families, as needed.
Essential Job Duties 
<ul>
<li>Serves as resident advocate when an issue arises that affects their daily lives or well-being.</li>
<li>Initiates 504 process for reasonable accommodations and modifications to address residents needs and manages process to ensure timeliness of completion. Works with the 504 Coordinator and maintenance staff, as needed based on approval or denial of accommodation by completing the process with a written notification to resident of the final resolution of 504 request.</li>
<li>Prepare a Supportive Services Plan. The plan must include: [1] activities and services that facilitate safely aging in place by helping participants proactively address their health and social support needs; [2] provide an ongoing benefit to maximize the impact of the funds; and [3] include the use of evidence-based programming (EBP) or evidence-informed programs.</li>
<li>Supports the Resident Services Supervisor in preparing and writing grants, proposals, applications, and reporting.</li>
<li>Provides case management by assessing resident’s need with self-disclosed care needs; updates support/needs in response to changing needs as residents age in place. </li>
<li>Develops resource directories for the housing community by working with property management team on outreach process and educates residents about community programs and resources.</li>
<li>Makes appropriate referrals to service providers on behalf of residents.</li>
<li>Creates annual calendars for ongoing education, wellness programs, compliance presentations for residents.</li>
<li>Liaison with community, creates awareness of the resident services program and develops partnerships to support the residents. </li>
<li>Support and coordinate care for discharged residents as they transition back to their apartments. Develop networks with family and friends to support residents.</li>
<li>Facilitate language services to residents with limited English proficiency via the approved company translation services. </li>
<li>Provides support to residents, to include eviction, crisis prevention, and conflict resolution.</li>
<li>A member of the on-site team to ensure residents understand regulatory and lease agreement requirements, i.e., annual recertification documents, REAC/NSPIRE unit inspections, and reasonable accommodation/ modification requests/ orders.</li>

RESIDENT SERVICES COORDINATOR- Tidewater Area, VA 
Burlington Capital Properties is currently reviewing applications for a Resident Services Coordinator in the Tidewater Area, Virginia. This role assists in building successful programs and services to meet the needs of the residents, while fostering good community relations to enhance participation from residents and community partners.
Qualified applicants should have previous experience in the social services field and a Bachelor's degree, preferably in a human services field. 
This full-time position requires availability Monday through Friday 8:00 AM to 5:00 PM.
The successful Resident Services Coordinator will:
<ul>
<li>Prepare, collect, and summarize surveys to determine the needs of the residents, and provide written reports as directed or requested</li>
<li>Develop a list of programs and services along with written descriptions, goals and objectives</li>
<li>Create, maintain and update listings of contact names, addresses and phone numbers of all business and community involved with the Center</li>
<li>Identify local community organizations, government, business, and other community leaders that can support or assist the center</li>
<li>Serves as on-site coordinator for all programs and oversees the day-to-day operations at assigned properties</li>
<li>Maintain a current understanding of technical skills that would be required of students to carry out class assignments</li>
<li>Provide written reports to the Director of Resident Services or management as required</li>
<li>Maintain the confidentiality of all residents</li>
<li>Enforce all Local, State and Federal Laws and Regulations</li>
<li>Maintain a program for safety at the Center</li>
<li>Assist with all programs and day-to-day operations </li>
</ul>
KNOWLEDGE, SKILLS AND ABILITIES:
<ul>
<li>Possess excellent prioritizing and organizational skills</li>
<li>Ability to use computer and basic office equipment required</li>
<li>Ability to communicate effectively and professionally</li>
<li>Demonstrated ability to initiate and maintain effective, cooperative relationships with low-income residents, community groups, service providers, business leaders, and local government</li>
<li>Experience in program development and implementation, with an emphasis on programs related to children, youth, and their families</li>
<li>Strong public presentation skills; excellent oral and written communication skills; strong organizational and coordination skills; and ability to juggle multiple goals and priorities</li>
<li>Demonstrated knowledge of ability to advocate, organize, problem-solve for families, youth, and/or the elderly</li>
</ul>
EDUCATION AND EXPERIENCE:
<ul>
<li>Bachelor of Social Work or related work experience required</li>
<li>Minimum of three years experience in social service work required</li>
</ul>

For this position you Must be a Registered Medication Aide “RMA” or Licensed Nurse.&nbsp; Be able to provide oversite, direct care services, assistance and support to residents and administration while providing a conduit between administration, direct care, medication aides and physicians to facilitate best care practices, service standards, community philosophy, and its mission.&nbsp; &nbsp; &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Some of this Position's Responsibilities Include:
<ul>
<li>Schedule nursing/direct care staff and facilitate coverage&nbsp;needs.</li>
<li>Direct, evaluate and supervise all RMAs and direct care staff and report to administrator satisfactory and unsatisfactory performance.</li>
<li>Maintain confidentiality of all resident information.</li>
<li>Assure nursing forms needed for documentation are copied and available.</li>
<li>Assist with education and in-service training to nursing staff.</li>
<li>Interpret existing policy and procedure to nursing staff, residents, families, and physicians.</li>
<li>Assist new nursing employees in following facility policies and procedures.</li>
<li>Coordinate Physician/NP site visits with prepared listing of resident issues/concerns or needs and follow up to facilitate efficient provider care and resident well-being, etc.</li>
<li>Review new orders to ensure orders, eMAR, and medications match and are complete.</li>
<li>Review documentation on resident admitted and compare physician orders with each resident’s plan of care to assure appropriate implementation of the physician’s orders and plan of care is occurring.</li>
<li>Maintain resident’s charts in appropriate location/file.</li>
<li>Audit&nbsp;medication carts monthly and as needed.</li>
<li>Create, implement and review Individual Service Plans (care plans or ISP) per regulations, routinely.</li>
<li>Complete “Uniform Assessment Instrument” (UAI) form.</li>
<li>Assist with the admission and discharge process of residents.</li>
<li>Report changes in condition of residents to administrator and physician.</li>
<li>Answer business phones and nurse call bells and respond to alarms as required.</li>
<li>Tour inquiring individuals or families when administrator or marketing/admission staff are not in facility.</li>
</ul>
Team Effort:
<ul>
<li>Develop and update each resident care plan/” Individualized Service Plan” (ISP) and UAI.</li>
<li>Assist in ensuring compliance with State and Local Regulations.</li>
<li>Follow instructions provided by the Administrator and provide appropriate feedback often.</li>
<li>Work with Administrator on strategies to improve quality of care and service to residents.</li>
<li>Work with the Administrator to develop training for staff and staff recognition, where appropriate.</li>
<li>Work with the facility Team leaders to provide consistent treatment and messaging to all staff, residents, and resident families for stability of the workplace.</li>
</ul>

<li>Identify staff issues and communicate them to the Administrator.</li>
<li>Keep the environment safe for residents, staff and guests.</li>

<li>Practice safety in working around and with equipment, chemicals, tools, and utensils.</li>
<li>No horse play or violation of the facility code of conduct.</li>
